If the applicant’s Photo ID does not include an address or the address does not match the applicant’s home address, they may submit a supplemental document in the Photo ID section or supplemental document section.


Acceptable Address Verification documents are below:


  • Federal, State, or Tribal Identification Card
  • Utility Bill
  • Income Statement or W-2
  • State, Federal, or Tribal Tax Return
  • Mortgage or Lease Statement
  • Official Letter from a Federal, State, or Tribal Government/Agency
  • Program Benefits Letter
  • Driver's License (Printed or Temporary is acceptable)

PROGRAM BENEFITS LETTER - Letter or Issued date not visible but has Eligibility date

If the Supplemental proof does not show the Issued Date but with Eligibility Date. The Supplemental proof is valid and acceptable.

PROGRAM BENEFITS LETTER - Applicant name found as the Eligible individual

If the Applicant's name found as the Eligible individual instead the Household's name in the supplemental proof.  This is valid and acceptable.
