Acceptable Supplemental Proof for D.O.B Mismatch

If the applicant’s DOB does not match the Photo ID, they will need to submit a supplemental document that matches the DOB in the application.


Acceptable documents must include the applicant’s full name or address & dated within the last 12 months or have a future expiration.

List of Valid Supplemental Documents:

  • Birth Certificate 
  • Driver's License with DOB (Printed or Temporary is acceptable)
  • Medicaid with DOB
  • Other Valid/Acceptable Supporting Documents that have DOBs

Birth Certificate/Certification of Birth 

- Accepted as supporting proof for DOB Mismatch if all other info is matched. It does not need a picture or issued date



Temporary Driver's License

- Accepted as supporting proof for DOB, Name, and Address Mismatch if all other info is matched.

Accepted supporting proof for DOB however, it needs an additional supplemental proof for address.
