ACP and Lifeline consumers are alive and eligible for benefits

Starting Tuesday 6/6/23, customers can upload documents directly through the My Benefit Status page.

Gen Mobile will be using a Master Index list to verify ACP and Lifeline consumers are alive and eligible for benefits.


To prove their eligibility they need to provide 2 types of documents.
1 document to prove their identity and
1 to prove they are alive. The documents will need to be mailed to Gen Mobile's PO Box(Gen Mobile, ACP

P.O Box 1187 Gardena, CA 90249). If a customer does not submit documents in 30 days, the account will be disconnected.  


Customers will receive the text messages below. 

Text Messages:

GenMobileFreeMsg(1/2): Your ACP service is at risk of being terminated. We need to verify your eligibility and proof of life.

GenMobileFreeMsg(2/2): Please visit https://www.genmobile.com/pages/proof-of-life for instructions on how to submit required documents or call 611 for more info

Documents Requested:

Documents to prove identity:

U.S. government, military, state, or Tribal issued ID;

Military discharge documentation;

Weapons permit;

Government assistance program document;

Statement of benefits from a qualifying program document;

Unemployment/worker’s compensation statement of benefits; or

Eligibility confirmation from state eligibility database with administrator.


Documents to prove you are alive:

(unexpired and dated within the last three months)

Current program eligibility or government assistance documentation;

Current utility bill (phone and internet bills cannot be used as utility bills; water, electricity, and waste and removal bills can be used as utility bills);

Current income statement such as a paystub;

Current mortgage or lease statement;

Current retirement/pension statement of benefits;

Current unemployment/workers’ compensation statement of benefits; or

A notarized letter affirming the subscriber’s identity and alive status.


If you do not have any of the documents listed above to prove your identity, please submit one document with your DOB and a second document with your SSN4.

The DOB document must be one of the following:

Driver's license;

Birth certificate;

Certificate of naturalization;

Proof of citizenship;

Permanent resident or permanent resident alien card; or

Passport.

The SSN4 document must be one of the following:

W-2 from within the last two (2) years;

Prior year's state, federal, or Tribal tax return;

Social Security card; or

SSA-1099.

How to upload documents in My Benefit Web Page

1. Click Submit proof of life now

2. Click Start

3. Click take Photo (to take photo using PC or phone) or Choose file (to upload the picture as file) to share the proof you are alive

4. If Take Photo was chosen, make sure photo is alligned with frame

5. If Upload File chosen, make sure the file is clear and legible

6. If you dont have such document, you could cancel the upload process and retake it later

7. Once file or picture is uploaded, you will be prompted with a preview of it to approve, if so click on Confirm

8. Once confirmed, file or picture will upload and you will see a confirmation message

9. Once file uploaded, proceed to take Photo or upload file to share the identity proof

10.Upload file or take picture

11. Wait for confirmation

12. If files are rejected, you will see a rejected message on My Benefit Status page

13. If files or pciture are approved, you will see a Approved message
