- SNAP - Must be a letter stating the person applying is receiving SNAP benefits.
- Medicaid - Medicaid - Can be a card or letter. Providers vary, but they should have Medicaid as the plan name. If the card or letter shows the customer is under the Star+Plus.
plan it is acceptable, but needs to also include the Medicaid Number.
- Health Benefit Coverage under Child Health Plan (CHIP) - Must be a letter stating the person applying is receiving CHIP benefits.
- Federal Public Housing Assistance - Letter stating the person is eligible to receive Federal Public Housing Benefits or a Federal Public Housing Benefits rental agreement.
- Supplemental Security Income - SSI - Must be a letter stating the person applying is receiving SSI benefits.
Benefit letters with a time frame must be valid during the time of the application. If the letter does not include a start and ending time frame, it must be issued within the previous 12 months or have a future expiration date.
- Letters must be issued by a state or federal agency and the agency must be visible on the letter. Either the name of the agency or a logo.
- Letters need to include the issuing entity, subscriber name, and address.
