Lifeline Only Tablet and BYOD

• The plan selection section will have an update to the plans that are available in different states. Some states provide additional funding and customers in those states get additional data. Customers who qualify for tribal plans also get additional data. (image below)
  

• The device selection option will have the option to get a free phone, get just a SIM/Bring Your Own Device or get a SIM and tablet. (image below)
  

*****The option says Tablet and Bring Your Own Device, but in this option they will be receiving a tablet and SIM.***** 

• In the benefit activation section they will enter the SIM and tablet IMEI. (Images below)

 THE FIRST OPTION SAYS PHONE, BUT WE SHOULD ENTER THE SIM, NOT A PHONE IN THAT SECTION.  

• In the last part of the enrollment section, it will go from pending to enrolled. If an agent calls to check the status, please advise that they should only provide the device when they see the application as enrolled. (image below).
