For Texas Lifeline the street type and any directional information must match. All other current exceptions can be made for address verification.
"If the address document reads 733 S 8th Street the application needs to include S, So or South and must be a street. If the address has an abbreviation for part of the address, we can still accept it"
- Unexpired driver's license
- Utility bill
- Current income statement from an employer, paycheck stub, or W-2
- Prior year's state, federal or Tribal tax return
- Current mortgage or lease statement
- Unexpired government, state, or Tribal issued ID
- Government assistance program documents
- Statement of benefits from a qualifying program which contains name and address of subscriber
- Retirement/pension statement of benefits
- Unemployment/workers' compensation statement of benefits
- Documentation containing the latitudinal and longitudinal coordinates of subscriber’s
Pictures of a picture or a screenshot are acceptable as proof of eligibility as long as they include all required fields. If the customer is using a screenshot from “yourtexasbenfits.com” it needs to include either the URL or the logo for it to be eligible.
